Applications are invited for engagement of Block ASHA Facilitator on purely contractual basis under ASHA programme for Bankura Sub-Division . The Monthly salary of Block ASHA Facilitator will be Rs. 7500.00 (rupees seven thousand and five hundred) only. In addition, he /she will be eligible for a monthly mobility support of Rs. 1500.00 (one thousand and five hundred) only. Block ASHA Facilitators will be contractually engaged for a period of one year renewable based on budgetary sanction and performance.

Qualification: Master’s degree in Social Science/ Sociology/ Social Anthropology / Social Work (MSW) / Business Administration (MBA) / Economics-/ Rural Development / Mass Communication
OR
Graduate degree in any discipline with minimum 2 years experience in health projects
Selection Process: The selection will be based on merit (academic result) and experience in health projects, written test and Computer Skill Test .Weightage will be given as per guideline vide G. 0 No – HFW / NRHM / 272/ 2010 / 875, Dated – 26/02/14.
Mode of Application:
1. Candidates should submit applications in the format annexed as “Annexure- A” to this notification. Application form will be downloaded from www.bankura.gov.in.
2.The application should be on good quality paper (Legal Size) using one side only.
3.Candidates should ensure that the application is made on a single sheet conforming to the above specification.
4.Candidates should fill up the application form in his/ her own handwriting in English with blue ball point pen only.’
5.Applicant must enclose the ‘following documents with the application form:-
-Â Self attested photocopy of certificate as proof of the Date of Birth. (Only Admit Card/ Board’s Certificate of 10th class will be accepted.)
– Self-attested photocopy of proof of residence (EPIC card/driving license/Bank Passbook etc/Electric bill/telephone bill etc).
– Self-attested photocopy of Mark sheet of Secondary or its equivalent examination.
– Self-attested photocopy of Mark sheet of Higher Secondary or its equivalent’ examination.
– Self-attested photocopy of Mark sheet of Graduation.
– Self-attested photocopy of Mark sheet of Master’s degree (if applicable)
– Self-attested photocopy of Computer Certificate.
– Self-attested photocopy of Proof of Experience (if any).
(Continuous engagement in a health related project with an assigned designation, during the *entire tenure furnished as experience will only be considered as “experience in health project”. No certificates specifying that the candidate has worked is volunteer e.g. in Pus Polio, social work, leprosy etc will be considered as experience in health related project.)
– Self attested photocopy of Caste Certificate from Competent’ Authority (for reserved category).
– Two self-addressed envelope pasted with postal stamp @ Rs.5 on each envelope.
– Demand Draft of Rs. 100.00 (Rupees One hundred) only for UNRESERVED categories and Rs. 50.00 (Rupees Fifty) only for RESERVED categories, issued from any Nationalized Bank in favour of “S.D.O. BANKURA.” payable at Bankura.
– A recent passport size colour photograph must be pasted on the application form.
– On the envelope of application containing the application, the applicant must write “Application for the post of Block ASHA Facilitator”.
